What to Expect During Your First Pelvic Floor Physical Therapy Session

Our first sessions are at least one hour long, though we also offer two-hour evaluations for complex cases. We begin by reviewing your specific concerns and gathering detailed information about your symptoms, lifestyle, and health history.

One unique aspect of our approach involves an osteopathic technique where our therapists will often lightly place a hand on your head and "listen" to your body to determine where your primary restrictions are located. Some might think it's a little "woo" but this valuable assessment tool helps guide our treatment approach from the very beginning.


Physical Assessment: Understanding Your Body's Patterns

We typically start with postural and movement assessments, observing how you move, breathe, and hold yourself. We'll test your strength and flexibility to identify any functional imbalances or holding patterns that might be contributing to your symptoms.

External assessment of your abdominal and pelvic organs helps us feel for restrictions or areas of tension. This hands-on evaluation provides crucial information about how your body is functioning.


Why Internal Assessment and Treatment is Beneficial

While some pelvic floor therapy can be done externally, internal assessment and treatment offers significant advantages for optimal outcomes. Working internally allows your therapist to get closer to the specific pelvic floor muscles and structures that need attention, providing much more precise and targeted treatment. This specificity means we can identify exactly which muscles are tight, weak, or not coordinating properly, rather than making general assumptions. Internal work also helps us locate the root cause of your symptoms—whether it's muscle tension, trigger points, scar tissue, or coordination issues—leading to more effective, personalized treatment that addresses your unique needs rather than just managing symptoms. It's important to know that we meet patients where they are and if there is hesitation around internal work, it's entirely your choice and you can refuse or change your mind at any point in the session. Your sense of safety and agency are a priority.


How The Internal Exam Is Performed

When it's time for the internal examination, your therapist will typically step out of the room to give you privacy to remove your underwear and provide you with a towel or sheet to cover yourself before returning. Some patients prefer to save time by having the therapist simply hold up a towel while they remove their underwear—this is completely your choice based on your comfort level. During the internal assessment, your therapist will use gentle, gloved fingers to evaluate muscle tone, trigger points, and coordination patterns. The examination typically begins with external assessment of the vulvar area to check for sensitivity, scar tissue, or muscle tension. For the internal portion, your therapist will slowly insert one finger vaginally to assess the pelvic floor muscles from different angles and depths. In some cases, a rectal examination may also be necessary to fully evaluate certain muscle groups, particularly if you're experiencing bowel-related symptoms. The examination is performed slowly and with clear communication—your therapist will explain each step and check in with your comfort level throughout. You maintain complete control and can ask to pause or stop at any time. During the assessment, you may be asked to contract, relax, or breathe in specific ways to evaluate how your muscles respond and coordinate.


Treatment Begins During Assessment

Unlike traditional medical exams that only gather information, our assessment process incorporates treatment from the start. If we identify tension in muscles, fascia, or organs, we may apply sustained pressure to these areas while incorporating breathing techniques and nervous system calming methods.


A Progressive Understanding

Because most patients have multiple factors affecting their symptoms, the assessment process typically continues over the first 2-3 sessions. This allows us to truly understand your body while your body becomes familiar with our therapeutic approach. Each session builds upon the previous one, creating a comprehensive picture of your unique needs.


Home Program and Next Steps

We often provide simple exercises or techniques to work on at home, especially after the first session. These are typically straightforward interventions that will make a significant difference by your next appointment.
